I, the undersigned participant (or parent/guardian if the participant is a minor), acknowledge and agree to the following:

1. Assumption of Risk. I understand that participation in fitness training, group exercise classes, and any related activities at or through Gaskill Group Fitness & Personal Training ("Gaskill Fitness") involves inherent risks of physical injury, including but not limited to muscle strains, sprains, joint injuries, cardiovascular events, and in extreme cases, death. I voluntarily assume all such risks.
2. Release of Liability. In consideration of being permitted to participate in activities at Gaskill Fitness, I hereby release, discharge, and hold harmless Gaskill Group Fitness & Personal Training, its owners, coaches, employees, agents, and representatives (collectively, "Releasees") from any and all claims, demands, losses, and liabilities arising out of or in connection with my participation, including those caused by the negligence of the Releasees.
3. Medical Fitness. I represent that I am in good physical condition and have no medical condition that would prevent me from participating in physical exercise. If I have any known medical conditions, I have consulted with a physician prior to beginning any training program. I agree to inform my coach of any medical conditions, injuries, or physical limitations before each session.
4. Emergency Medical Treatment. I authorize Gaskill Fitness and its staff to seek emergency medical treatment on my behalf (or on behalf of the minor participant) in the event I am incapacitated and unable to give consent. I agree to be financially responsible for any medical expenses incurred.
5. Personal Property. I acknowledge that Gaskill Fitness is not responsible for the loss, theft, or damage of any personal property brought to the facility.
6. Photography & Media. I grant Gaskill Fitness the right to use photographs or videos taken during sessions for promotional purposes, including social media and the company website, unless I notify staff in writing that I do not consent.
7. Rules & Conduct. I agree to follow all facility rules, coaching instructions, and gym etiquette policies. I understand that failure to do so may result in termination of my membership.
8. Indemnification. I agree to indemnify and hold harmless the Releasees from any claims, causes of action, or liability arising from my participation or the participation of any minor under my supervision.
9. Governing Law. This waiver shall be governed by the laws of the State of Washington. If any provision is found to be unenforceable, the remaining provisions shall remain in full effect.

I have read this waiver carefully. I understand that by signing below I am giving up legal rights and remedies. I sign this freely and voluntarily.
